This leak adjustment request must be filled out by the owner of the property. If you are a tenant, please contact the owner and ask them to fill out this form. Please note, this is a one-time leak adjustment for each utility account, amounting to 50% of the monthly bill impacted by the leak. The customer must make the leak adjustment request within one month of the leak discovery. To receive the adjustment, the account holder must also show proof the leak has been fixed. Please read Water Leak Adjustment Resolution below and check all applicable boxes before submitting this form.
